WHO WE ARE
Support Bridge Care Network provides practical, respectful and person-centred NDIS support for adults with disability who value independence, dignity and reliable support.
We take the time to understand what matters to you and work with you, your family and other professionals to make support clearer, more coordinated and more meaningful.

WHO WE SUPPORT
We work alongside adults (19–65) with low to moderate support needs, including:

✓ Early to mid-stage neurological conditions (e.g. MS, Parkinson’s)
✓ Sensory disabilities (e.g. vision or hearing impairment)
✓ People living independently or semi-independently
✓ People who want practical support without being over-managed
